The only time that V8 would be entered more than one time on a claim would be when there are two confirmed access infections in a given month with a break in antibiotic therapy. If there is a break in the administration of antibi-otics followed by another positive cul-ture within the same billing month, then V8 should be entered on the second date as well. CMS offered this sample write-up:"Infection 1 culture obtained March 1 and was positive for infection; patient treated for two weeks with antibiotics, antibiotics stopped, and then a week later (March 26) another positive culture was obtained." The provider would enter V8 for this date as well. All other dates during the month would have a V9, CMS said.
